A typewritten letter explaining why you failed to meet the SAP
requirements and a description of what has changed to allow you to
meet the requirements in the future.

Documentation that reflects the situation you are describing in
your SAP letter.

A signed SAP Plan of Action Memorandum of Understanding.

need to complete the SAP Academic Plan of Action Memorandum of
Understanding.

I understand SAP appeals submitted with incomplete paperwork will
be returned to me.

I understand changing my major can affect my eligibility for
financial aid as I may not be able to maintain pace progression
and complete my credential within my maximum time frame.

I understand that I am responsible for any tuition payment due
while my appeal is being processed. If my appeal is approved, I
understand my SAP issue could delay the awarding process. If my
appeal is denied, I understand I am responsible for payment of
tuition charges.

I understand it is my responsibility to check my Student Self
Service to check the status of my financial aid.
*
I understand that I am responsible for ensuring the completion of
all other components of my financial aid file.
*
I understand the SAP Appeal Committee may defer ruling on my
appeal pending the need for additional documentation and this may
delay the process.
*
I understand my program of study/credential must require at least
16 credit hours for completion AND at least 15 weeks of study for
completion for me to be considered for financial aid.
*
I understand Financial Aid will not pay for courses taken by
audit, credit hours earned by placement tests, or nondegree
courses.
*
I understand transfer credit hours count in the
attempted/completed SAP hours as recorded by Admissions and
Records.
*
I understand students may only receive funding for remedial
coursework for a maximum of 30 attempted credit hours.
*
I understand students may only receive funding for coursework
while declared program is undecided or pending for a maximum of 30
attempted credit hours.
*
I understand the SAP Appeal Committee may approve my appeal with
Academic Progress conditions or limit future hours.
*
I understand the decision of the SAP Appeal Committee is FINAL.

false or misleading information on any financial aid documents, I may
be fined, sentenced to jail, or both.

SAP Appeals must be submitted to JCTC’s Financial Aid Office within
the following deadline date for review:

future semesters.
